You should always contact your insurance provider to verify we are in network and if authorization is needed!

• Blue Cross/Shield PPO

• Blue Cross/Shield HMO- All groups besides Amita and Loyola will need an authorization from your primary care provider
• Blue Cross/Shield Community Health
• Aetna PPO
• United Health Care
• Cigna, higher copayment will apply

We are also in other networks, please check with your insurance company using the individual provider's name such as Mitchell Glaser MD

Currently we are not accepting Illinois medicaid but the out patient clinic at Ascension St. Mary's Medical Center does. They deliver excellent treatment and we provide clinical services to their programs. For appointments call (708) 410-0615

For in patient hospitalizations and IOP services most insurances are in network with us in addition to the ones above.
